We are pleased to announced that you can now access health services from Grange Medical Practice on the new NHS Wales App.
NHS Wales App:
Through the app you can:
- Order repeat prescriptions
- Access your medical summary
- Get NHS 111 Wales health advice
- Blood and organ donation information
You will need a fully verified NHS login, or a valid photo ID to set one up. Select 'Grange Medical Practice' and follow the instructions given.
My Health Online
If you are under 16, or do not have a valid photo ID, My Health Online is still currently active as normal.
Sign up to My Health Online by completing the Registration Form below and send it back to the practice via email reception.w97061@wales.nhs.uk

The surgery will send you a confirmation letter via return email with your registration details. You can then book your Flu appointment online.

Who can have a Flu Vaccine:
- Patients aged 55 and over.
- Patients aged 6 months to under 65 years in a clinical risk group:
Asthma
COPD
Bronchitis
Chronic Heart Disease
Chronic Kidney Disease
Chronic Liver Disease
Chronic Neurological Disease
such as Parkinsons or Motor Neurone Disease
Diabetes
Learning Disability
Asplenia or Splenic Dysfunction
Obesity
Weakened Immune System (Immunosuppressed)
Carers
Who can have a Covid Vaccine:
- Patients aged 65 and over.
